How the STAR Program Works
Four stages of STAR Program Implementation:
- Assessment – New STAR Programs participate in a thorough gap assessment generated after careful review of referral patterns, analytic cancer caseload, patient population data and other factors. Customized recommendations are developed based on the findings.
- Train the Team — STAR Programs access extensive educational materials, including 10 online modules that summarize current literature and evidence-based best practices. In addition to cancer rehab, topics such as navigation, distress screening and survivorship care plans are also addressed. Training is designed for busy clinicians and is CEU compatible.
- Implement Protocols — STAR Programs implement evidence-based protocols. Each protocol includes specific tools, procedures and resources designed to improve outcomes in one of the four targeted areas. Protocols are aligned during implementation with four key outcome categories: Clinician Education, Patient Functional Outcomes, Patient Satisfaction, Referrals and Revenue.
- Measure Outcomes and Accelerate Success – STAR Programs routinely analyze and report outcomes to their executive leadership and the STAR Program Oversight Committee. STAR Programs stay on track with quarterly progress reports, annual reviews and on-going support from their assigned Clinical Consultant. STAR Programs Steering Committees review results and, with support provided by their clinical consultant, help direct the program strategy.
The Team
Implementing a successful STAR program requires thoughtful leadership and a team approach. Key members of your STAR Program will include:
Oncology Rehab Partners
- Advisory Team – provide expertise, guidance and ongoing strategic support to your facility’s STAR Program Steering Committee
- Clinical Consultants – work closely with oncology and rehabilitation department leaders to implement protocols and operationalize the cancer rehab service line
Client Organization
- STAR Program Steering Committee – Interdisciplinary team of strategic leaders charged to direct the cancer rehabilitation service line
- STAR Program Coordinator – clinical first-line supervisor at each facility who serves as the onsite program leader for their respective department
- STAR Program Team Members – PTs, OTs, SLPs, Nurses, Navigators, Oncology and Rehab Service Line Directors, mental health professionals and others engaged in the implementation and operation of the cancer rehabilitation service line
